An Interview with Dr. Nicole Patel on Breast Augmentation

Dr. Nicole Patel is a cosmetic surgeon boarded by the American board of surgery. Here the Doctor has answered some of the common questions Bizymoms visitors have about Breast Augmentation.

 


Q.     What is breast augmentation?


A.    Breast augmentation is a procedure to enhance or augment the size of one’s breasts.  This may be with an implant or with autologous fat transfer (fat transfer using one’s own fatty tissue taken from another location).  A lift procedure may be done at the same time or as a separate (preceding) surgery.


Q.     How should one prepare for breast augmentation surgery?


A.    There is an abundance of information available on the internet regarding breast augmentation. It is important to read from multiple sites and create question lists prior to going for a consultation. It is also a good idea to arrange consultations with several surgeons, to increase your knowledge of the options available and compare recommendations. Second visits may be necessary should you have additional questions. Keep in mind that you will ultimately have to choose the type (saline vs. silicone) of implant to be used as well as the incision approach and the desired size; this will be decided after you have discussed your individual anatomy and desires with the surgeon. Consider plans for future child-bearing and importance of breast feeding before having breast implants. Finances of initial surgery and possible complications should also be considered before having surgery.


Q.     How is a breast augmentation surgery performed?


A.    Breast augmentation surgery can be performed in multiple techniques depending on the incision approach, placement of implant and associated procedures performed (such as a lift or liposuction).  Typically, an incision is made under the breast (inframammary), around or within the areola (peri or intra-areolar),  in the underarm (transaxillary) or in the belly button (transumbilical – only an option for saline implants).  Additional incisions may be needed if your nipple position needs to be elevated to obtain the best aesthetic outcome. Often liposuction of the anterior axillary region(fatty tissue which protrudes between a woman’s bra strap and her underarm) is performed with the augmentation to give a better result. Placement of the implant above or below the muscle is another factor to be considered. Most often it is preferable to have a submuscular placement.
Fat transfer to the breasts is an option for some women who do not desire an implant, have adequate fatty deposits elsewhere to harvest the fat from, and who do not want a significant increase in their breast size.  In some countries outside of the USA there are also fillers which are used to augment the breasts; this is not FDA approved here.
The type of anesthesia utilized for this procedure varies from one surgeon to the next, but I recommend you seek a board certified anesthesiologist to perform the anesthesia.


Q.     What are the possible immediate post-operative effects in breast augmentation surgery?


A.    There are both immediate and long term risks to breast augmentation surgery, including infection, bleeding, swelling, change of implant position, capsular contracture, scarring, and  sensory changes to name a few.  A deep venous thrombosis (DVT) or blood clot with the potential for a pulmonary embolism is the most dangerous risk.


Q.     What are the possible longer-term, local effects in breast augmentation surgery?


A.    Capsular contracture is the most important long term effect to be aware of and understand.  Implant rupture is another possible short or long term complication.  Also, gravity will continue to play a role in the appearance of your augmented breasts over time, as will changes in weight and becoming pregnant.


Q.     What is the recovery time for breast augmentation?


A.    Recovery time is 1-2 weeks, but full wound strength is not achieved until 8 weeks. This means you will have activity restrictions which your surgeon will discuss in detail with you.  Gradual increase in activities will take place after 2-3 weeks, with no strenuous activity for 6-8 weeks.


Q.     How much does breast augmentation cost?


A.    Price varies from one surgeon to another and from one location to another.  Typically $4000-$9000, depending on implant (saline or silicone) and simultaneous procedures performed, such as liposuction or lift procedure.  Be sure to inquire about anesthesia and facility charges as well as any possible pathology charge.  Prescription medications are typically not included in your price quote.
